系列第4作，於1998年8月的Comic Market 54正式發售。此遊戲加入了「低速模式」和「擦彈系統」系統，以讓玩家更容易躲避子彈。另外，玩家亦可以選擇不同的自機。至此，系列往後作品的特徵已大致具全。此遊戲包含六關卡並追加Extra關卡。根據ZUN本人指出，《東方幻想鄉》在Comic Market54僅售出了約200至300份遊戲。
